Terms and Conditions - when purchasing any membership product linked with our 24hr Emergency line service (jewellery items, engraved items or other membership specific items).

1. I acknowledge that Emergency ID Australia may release any of the information provided on the membership form to any person Emergency ID Australia reasonably believes to be medical or emergency services personnel.

2. I acknowledge that all medical or miscellaneous information provided to medical or emergency staff will be provided to them as a guide only and Emergency ID Australia will not be liable for information provided or omitted, and information is only relayed from membership information provided to Emergency ID Australia.

3. I acknowledge that Emergency ID Australia shall not be liable for any injury, loss or damage sustained by me or by any other person arising directly or indirectly as a result of the provision of any information or omission of any information by or on behalf of Emergency ID Australia.

4. I acknowledge that it is my responsibility to check the information is correct on my medallion, and to ensure that I immediately advise Emergency ID Australia if I become aware of any inaccuracies, changes or incomplete information that I have provided.

5. I agree that the products Emergency ID Australia provide are intended to be worn by the member concerned only, and that each engraved item is unique and cannot be transferred or worn by any other person.

6. I release and indemnify Emergency ID Australia from any claim, legal or otherwise by me or any other person arising directly or indirectly as a result of providing information to Emergency ID Australia.

7. I release and indemnify Emergency ID Australia from any claim and for any liability for providing information to any third party without my consent that Emergency ID Australia reasonably believes to be in my best interests.

8. I will indemnify and keep indemnified Emergency ID Australia if Emergency ID Australia suffers any liability, loss or damage if the information is incorrect, incomplete or out of date due to my failure to notify Emergency ID Australia of the correct information.

9. Emergency ID Australia will provide a record of the information held by Emergency ID Australia to the intended member for review. I will review and promptly advise Emergency ID Australia of any changes that need to be made.

10. I acknowledge that the principles set out by the Privacy Act 1988 (Cth) apply to Emergency ID Australia.

11. If I am signing this application on behalf of another as a next of kin, carer, parent, guardian, family member, or purchaser, I warrant that I am authorised to complete this application on behalf of the member and that the information is complete and accurate. I agree to indemnify Emergency ID Australia from any current or future claim, cost, loss or damage suffered by Emergency ID Australia as a result of a breach by me of this warranty.

12. I understand that if the person I am purchasing this service for is over 14 years of age they will receive the order and confirmation letter in order to enable the person I am purchasing the service for to review and update any information retained by Emergency ID Australia. I acknowledge that if the person I am purchasing this service for is under 14 years of age it the responsibility of a parent or guardian to ensure the information is current and accurate.

13. I have read all the information on this membership form and have completed it accurately and in good faith, and have read and agree to be bound by the above terms 1 to 12.
